What are the Georgia Low THC Oil Registry and Card?

The Georgia Department of Public Health manages the Low THC Oil Registry to provide a personalized Low THC Oil identification card to qualified Georgia patients or their caregivers.

The Low THC Oil Card allows a patient to possess 20 fluid ounces of low THC oil to treat a qualifying condition. For the list of these conditions, go to https://dph.georgia.gov/low-thc-oil-registry and click on FAQs for General Public.

This effort is dictated by Georgia House Bill 1.

What’s the process to request a card?

The process is as follows:

1. The physician treating the patient for the qualifying condition registers with the Georgia Low THC Oil Registry here: https://sendss.state.ga.us/ords/sendss/!thc.thc_login. The physician also submits these forms:
   • Low THC Oil Physician Certification Information
   • Low THC Oil Waiver The patient (and one or two caregivers, if desired) must sign this form and have it notarized.
2. If the registration is approved, the Georgia Department of Public Health’s Low THC Oil Unit prints the card. The unit sends the card to the patient’s designated county Vital Records Office.
3. Once the Vital Records staff receives the card, they let the patient or caregiver know it’s ready for pick up.
   The patient or caregiver must bring:
   • Their valid identification (ID), such as a driver’s license or state-issued ID.
   • $25, payable in cash or by credit card, certified check or money order.

A caregiver must also bring:

• • A completed [Authorization for Release of Low THC Oil Card].
  • The patient’s ID.
