SERVICES DURING THE COVID-19 PANDEMIC
All clients and visitors must:
- wear a mask
- get their temperature checked with a thermal temperature scanner
- complete a COVID-19 screening questionnaire
We will accept walk-ins based on availability. For a quicker visit, call (404) 244-2200 to schedule your appointment.
You may not bring additional people to your visit unless you have a physical or mental limitation, need an interpreter, or are a parent or guardian accompanying a child. We will allow 1 visitor per client.
To prepare for your visit:

- bring your insurance card and photo ID

Service Fee
We accept cash, checks (local banks ONLY), debit and credit cards. Insurance accepted only from State Health Care Benefit Plan, United Health Care, Cigna HealthCare (State Employees only), Medicaid and Medicare. Some of our service fees are based on a sliding scale. Unless arrangements have been made in advance, payments and any outstanding balances are expected at the time of service. Self-Pay: If you do not have medical insurance, you will be considered a Self-Pay patient. Self-Pay patients must pay for the medical services received at the time of the visit. Sliding Scale can be applicable for some services based on your income. We accept cash, personal checks (local banks ONLY), debit and credit cards.
Medical Records
Medical records are kept confidential. We will not release your medical information without a signed authorization from you.

- Complaints or concerns are documented, investigated, and acted on by the Supervisor or designee in the department where the complaint originated
- Presentation of a complaint does not in itself serve to compromise a patient’s future access to care

Back to School (Hearing and Vision /Dental Screening)
- Walk-in clinic for back to school immunization and health certificates.
- To have information transferred to a Georgia 3300 form, parents need to bring a copy of physical exam done in the last year, prior to school enrollment with hearing, vision and dental screenings clearly marked.

Pregnancy Testing
No appointment is necessary. There is a $50 prepayment for services. If the pregnancy test is negative, there is a fee for service. Your visit will include:
- Urine for a pregnancy test
- Finger stick for lab
- Medicaid application (one of the following documentation is needed: Birth Certificate, USA Passport or Naturalization Certificate, or I94/Green Card)
- WIC referral
- An appointment for Perinatal Case Management

Prenatal Case Management
- To improve birth outcomes by assisting pregnant Medicaid eligible women in gaining access to needed medical nutritional, social, educational other services.

STD
- Walk-in clinic for STD evaluation/treatment and HIV testing
- Fees are based on income using a sliding scale. You will be asked to pay $50 prepayment before service. Fees will be discussed with you by the cashier.
To help speed your visit with our STD Clinic, you may download and print the following documents (PDF format,) fill them out, sign where appropriate then bring them with you:
- Print out the Registration Form then fill in the blanks and sign the bottom.
- Read the online Notice of Privacy practice.
Breast and Cervical Cancer Program
The Breast and Cervical Cancer Program was established to provide screening and follow-up services to low income, uninsured/underinsured women throughout the state of Georgia because “Early Detection Saves Lives”.
Basic Eligibility for the Breast and Cervical Cancer Program:
- Women 50-64 years of age
- Women 40-49
- With a family history of breast cancer (mother, sister, aunt) experiencing problems
- DeKalb County Resident
- Uninsured or Underinsured
- Income less than 200% of poverty:
