School Screenings

We provide the screenings required for admission to childcare, Head Start, pre-kindergarten, and kindergarten through 12th grade.

The screenings assess:

• Immunization status
• Hearing
• Vision
• Dental health
• Nutrition
• Scoliosis

We also complete these required Georgia Department of Public Health forms for childcare and school admission:

• Certificate of Immunization (Form 3231)
• Certificate of Vision, Hearing, Dental, and Nutrition Screening (Form 3300)

Appointments are preferred. Walk-ins are welcome.
