• Children and teens from birth to age 20 years with chronic medical conditions such as (but not limited to) asthma, cystic fibrosis, diabetes, seizures, spina bifida and sickle cell anemia

• Offering diagnostic testing and care from specialists
• Providing durable medical equipment, medications and supplies
• Assisting with transition from pediatric to adult health care
